This is the first form needs to be filled out in order to start the burial process. You can speed up the process by downloading and filling the form before you head to the Cemetery.

Mortuary Form

Khadeeja Cemetery works with different mortuaries. If you went to a mortuary directly then you will have to fill out their form at their office. Or the second option is to let us arrange your loved one pickup from a hospital or your house to Cemetery and then you will need to fill this form when you come for burial.

Headstone

Headstones are optional. You can order one directly or pay at Cemetery and we will order one for you. 

Burial Purchase Form

Use this form to purchase a burial for you or family in advance so that you can reserve your place with your loved ones.
